Defib Rentals Agreement
(Unit information given will be given once agreement has been signed and unit allocated to hirer)

The lessee will have insurance to cover the cost of the units in case of loss or damage, or in the event the units are not able to be returned to the Onset Health Services Pty Ltd in the good working order they were received. If a defibrillator unit is damaged, lost or stolen, or cannot be returned to the Onset Health Services Pty Ltd in good order, the lessee agrees to pay for damages or replacement up to the value of $2,795.00 per unit. The amount to be paid will be determined by the Defib Shop Pty Ltd. See section below for signing.
GST is applicable to all transactions for the purposes of leasing agreements.
- VALIDITY TERMS:
Once full payment has been received, and this agreement is signed and unit delivered, the lease will commence. The lessee will notify Onset Health Services Pty Ltd if an extension is required, as early as possible. There are no penalties or extra charges above normal lease charges if reasonable notice is given.
- OPERATION OF UNIT:
The lessee will ensure that only currently qualified and authorised staff or volunteer(s) use the Defibrillator unit. Onset Health Services Pty Ltd accepts no responsibility for the use of the unit by any person. The lessee is responsible to ensure any units are used in a safe workplace environment.
The units supplied come with a set of consumables, including adult defibrillation pads, batteries and a patient preparation kit. Should the provided consumables be used or otherwise opened and or depleted, it is the responsibility of the lessee to purchase and replenish stock of the required items. Consumables are available from the Onset Health Services Pty Ltd.
- PAYMENT OPTIONS: YOUR PAYMENT IS MADE VIA YOUR ONLINE CART.
7.SHIPPING: Shipping included – delivered by Onset Health Services Pty Ltd couriers – Shipping via regular Courier – Road Freight will be arranged by the Onset Health Services Pty Ltd. The cost of freight to the lessee’s address will be charged to the invoice. Express Post will be charged additional relevant charge
The return shipping is to be arranged and paid for by the lessee. Any delays in return of the unit will be charged accordingly. The Onset Health can arrange for return pick-up and delivery at an additional cost of $199.95 per unit. Call 0487 454 445 to arrange.
Extension of hire charges apply if the units are not returned to the Onset Health Services by COB 3 days after last date of hire agreement. Up to 5 days are allowed for returns from SA, WA & NT.

As per the details above in point 1, the lessee will provide adequate insurance to cover the cost of the Defibrillators and the cases they are housed in, in the event of damage, loss or being stolen. The units are to be returned to the Onset Health Pty Ltd in good working order as they have been received, including the original defibrillation pads and batteries the unit was supplied with.
I understand that if the unit or units are damaged, lost or stolen, the lessee will pay up to the amount of $3,295.00 per unit as determined by the Onset Health Pty Ltd to replace the Defibrillator unit(s).
CREDIT CARD DETAILS MUST BE SUPPLIED WITH ALL CONTRACTS. IF YOU DO NOT PROVIDE CREDIT CARD DETAILS, YOU WILL NEED TO SUPPLY A BOND BY EFT OF 2X THE HIRE / LEASE TOTAL AMOUNT.
PENALTY FEES – AN OUTLINE OF GUIDELINES, FEES AND CHARGES:
PENALTY FEES AND IMPLEMENTATION DATES / PREIODS:
Onset Health allows up to 3 days either side of your booked period free of charge, or up to 5 days for SA, WA & NT hires. Firstly, for us to get the unit to you, and then for you to return the unit after your event.
- You hire / lease the defibrillator from the 10/03/2022 to the 24/03/2022
- Onset Health sends out to you approx 05/03/2022 – but you do not incur a fee for the period from 05/03 to the 10/03.
- You are then required to return the defibrillator to us by COB 27/03 – but you are not charged a fee for the period of 25/03 to COB 27/03.
If you return the unit after the COB 27/03, you will then be charged an administration fee of $75.00 + the weekly fee for each week or part thereof that it takes for you to return the unit.
Where you have hired / leased a defibrillator for the period of a weekend, if you have not returned that unit by the COB the Thursday following, you will be charged another weekend fee + an administration fee of $75.00.
WEEKLY ADMINISTRATION FEES:
ALL PENALTY FEES INCUR AN ADMINISTRATION FEE OF $75.00 PER WEEK PER UNIT UNTIL THE DEFIBRILLATOR(S) IS RETURNED. THIS FEE IS WEEKLY OR PART THEREOF.
RENT / LEASE FEE CHARGE:
FOR EACH WEEKLY BLOCK THE DEFIBRILLATOR(S) IS OUTSTANDING, THE SAME WEEKLY FEE (OR EQUIVALENT) WILL BE CHARGED PER UNIT.
FAIR & REASONABLE:
The Onset Health asks you to understand we are a reputable, fair & reasonable company to deal with. We understand there may be unforeseen delays or the need to extend a hire / lease period. If you know there is a delay to occur, or you would like to extend the period, call us earlier rather than later, or rather than not at all!
If you call us, we can discuss solutions to any problem you may have encountered or we can amend the documents and relevant fees to extend the period until the unit(s) is due back. In this case, penalties are unlikely to occur. Remember, we want this to be a positive experience so we are both happy to assist each other again in the future!!
fulfil the terms of this lease between the lessee and the Defib Shop Pty Ltd.
My position with the lessee is: as above and I declare I am in a position to authorise the above agreement on behalf of the lessee.
I understand this agreement will become effective once the above terms have been agreed to, and payment as per the invoice has been received by the Onset Health Pty Ltd (unless otherwise arranged).
